Carers Wellbeing Support Service

The onset of the Caring role can in itself be difficult, this may occur without warning such as caring for a loved one following an accident, or could involve Caring for someone with a congenital disorder.

 

Equally the loss of the Caring role through death or the Cared for moving into long term Care can have instrumental effects on the Carer.

 

At whatever stage the Caring role can be demanding and have a huge impact on a Carers life.

 

The transitions of the Caring role can often mean that Carers can become isolated with many Carers failing to address their own health and wellbeing needs.

 

The Carers Wellbeing Support Service provides a holistic support package enabling Carers to feel better supported and able to make informed choices about their caring role and own health and wellbeing.

 

Once a Carer is referred for support they will be allocated a named Carers Support Advisor who will provide the Carer with a My Caring Journey booklet working with the Carer to complete the booklet, produce a Carers Support  Plan and go through a wellbeing check, and also to provide the Carer with support, information and advice throughout the Caring role.

 

The Carers Support Plan will assess the needs and tailor a holistic support package for the Carer’s personal needs at varying stages of Caring including when Caring ends.

 

Carers will also be offered :

 

  • One to one support, including outreach work to reach the most isolated Carers in our community

  • One to One Counselling

  • Financial help and advice around benefits

  • Group Support through the operation of Carer Led weekly support groups, including specific support groups

  • Carers will received a My Caring Journal 

  • Carers Health and Wellbeing Support Groups

  • Referrals to other specialist organisations.

  • Support to apply for external funding and benefits
